We support people with enduring mental health problems.
We enable people with mental health needs to fulfill their potential on a long term basis. Person centered planning enables us to undertake detailed care and relapse prevention planning whilst measuring progress by working towards a set of realistic outcomes. These outcomes are identified by the people we support, which is a fundamental part of the recovery process.
Once we have been chosen to provide support, service users are involved in many aspects of planning their services which include training and recruiting staff. Our staff uses Pro-active Risk Management to support initiatives for service users to become active members of their communities, including employment and education. In our experience, this not only helps to increase social and emotional well-being but also decreases the stigma that can be attached to people with mental health needs.
We have an excellent working relationship with community mental health teams, local council, both private and voluntary organization and a proven track record of re-integrating people back into local communities. By working closely with community teams we are able to provide consistent and appropriate responses if there are periods of deterioration and in doing so prevent revolving door admissions to in-patient services.
As part of our comprehensive training programme our staffs undertake specialist mental health training which enables them to support people with mental health needs to take control of their own lives.
